The Chief Operating Officer (COO) reports directly to the CEO, President and holds accountability to the Board for all Service Delivery operational matters. The COO will provide strategic and operational leadership across a continuum of post-acute care services, including home care, hospice, and facility-based long-term care and skilled nursing. The COO will ensure the delivery of high-quality, patient-centered care while driving operational excellence, regulatory compliance, sustainable growth and profitability across all service lines and locations.

Responsibilities:
- Operational Oversight
- Lead and collaborate with Service area-heads to oversee operations across Home Care, Hospice, and Skilled Nursing facilities.
- Direct the achievement of organizational goals, including quality patient care, employee engagement, census growth, revenue, and net income through ongoing enhancement of internal operations and external service delivery processes.
- Drive innovation and standardize workflows and care protocols to ensure consistency, safety, and quality across all service settings.
- Oversee all resource allocation to meet patient needs efficiently in each service area.
- Strategic Leadership
- Collaborate with the CEO and executive team to develop and execute strategic plans for service expansion, geographic growth, and operational integration.
- Identify and implement opportunities for service line optimization and cross-functional collaboration, with a view to reducing costs and maximizing revenue.
- Lead initiatives to enhance patient experience, reduce readmissions, and improve care transitions.
- Regulatory & Clinical Compliance
- Ensure compliance with federal, state, and local regulations including CMS, DOH, and accreditation bodies (e.g., Joint Commission, etc.).
- Partner with clinical leadership to maintain high standards of care, infection control, and safety.
- Monitor and respond to changes in reimbursement models, including value-based care and managed care contracts.
- Financial Management
- Work with the CFO to develop and manage budgets, monitor financial performance, and implement cost-control strategies across the continuum of Service Delivery.
- Collaborate with Finance on financial market trends and growth projections
- Evaluate and negotiate favorable vendor contracts and service agreements.
- Identify cost control and optimization strategies
- Allocate resources strategically to align with company goals.
- Technology & Innovation
- In collaboration with the CFO, SVP of Information Systems and Chief Strategy Officer, drive adoption of appropriate health IT systems including EMRs, remote monitoring, and telehealth platforms.
- Support data-driven decision-making through performance analytics.
- Lead digital transformation initiatives to improve operational efficiency and patient engagement.
- Leadership & Culture
- Capacity to lead organizational change and transformation.
- Ability to inspire through a balanced approach that combines authority, persuasion, dialogue, inspiration, analytical thinking, and experience.
- Develop and maintain strong working relationships with physicians and other health care professionals.
- Build and mentor a high-performing operations leadership team across all service lines.
- Foster a culture of compassion, accountability, and continuous improvement.
- Promote interdisciplinary collaboration and staff development.
- Manages performance of direct and indirect reports. Provides orientation, coaching, training, and development. Take corrective action when necessary.
- Utilizes corporate services resources such as Human Resources, Compliance, Legal, Finance, Information Services, Marketing, etc., as appropriate.
- Ensures appropriate staffing and resources to support department/agency services. Monitors productivity throughout the year and participates in annual budget preparation.
- Interviews and participates in the selection process for qualified staff in collaboration with Human Resources and department management.
Qualifications:
- Master’s degree in Healthcare Administration, Business Administration, Nursing, or related field (MBA, MHA, MSN preferred).
- 10+ years of senior leadership experience in post-acute care, including home care, hospice, and/or long-term care.
- Demonstrated success in quality improvement and patient satisfaction initiatives (CMS 5-star quality programs).
- Extensive understanding of regulatory requirements, clinical operations, and reimbursement structures in post-acute settings.
- Proven ability to lead multi-site operations and manage complex organizational structures.
- Strong communication, strategic planning, and change management skills.
- Ability to lead in a mission-driven, patient-centered environment.
- Experience with Medicare/NY Medicaid reimbursement and value-based payment models in provider-based programs.
- Familiarity with EMR systems.
