On-Call Triage LPN - Team members in this hospice nursing job serve as a liaison between Hospice Home Care Planners, Social Workers, and RN Case Managers and RN on call Triage Nurses. They also respond to Social Work/RN Case Managers/patients/customers complaints and grievances.

Hospice Care Planner - Team members in this hospice nursing job are responsible for evaluating and establishing a plan of care for patients referred for home health services. The Home Care Planner will facilitate admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members.
