Social Worker - Team members in this social work job provide psycho social support and other Social Work services to Hospice patients and their families in the residence home care setting; both in the pre and post bereavement phase of Hospice service.

Social Work Case Manager - Team members in this social work job provide telephonic assessments of members’ medical, psychosocial, physical and spiritual needs. In close collaboration with an assigned Nurse Practitioner, they develop, implement, and monitor care plans, and ensure continuity of care across all healthcare settings.

Fee-for-Service Social Worker - Team members in this social work job assess patient psycho-social issues to ensure adequate delivery of home care services in order to meet agency goals and required objectives; and provide supportive counseling in regards to emotional, psychological and concrete needs including discharge planning.
