The Admission Nurse is a clinical professional responsible for initiating and resuming high
quality home health services for patients referred to the agency. This role conducts comprehensive home assessments to determine skilled care needs, establishes an individualized plan of care in collaboration with the interdisciplinary team, and ensures timely, accurate documentation that meets regulatory, payer, and agency requirements.

The Admission Nurse provides patient and caregiver education, evaluates home safety, and confirms eligibility for services. Through strong clinical judgment, excellent communication skills, and a patientcentered approach, the Admission Nurse helps ensure a smooth transition into home care and sets the foundation for positive patient outcomes.

Responsibilities:

Qualifications:

•    Associates Diploma in Nursing, required; Baccalaureate Degree, highly preferred.
•    Current RN License in NY (in good standing).
•    Minimum of one year of clinical experience in a Certified Home Health Agency.
•    Knowledge of Microsoft Office and experience documenting in a clinical software system preferred.
•    Driver’s License required in all boroughs except Manhattan.
•    Strong assessment skills, clinical judgment, and critical thinking abilities.
•    Ability to work independently in a community setting.
