This field-based position will cover the following locations: Northern Brooklyn, Lower Half of Manhattan and The New Jewish Home.

Responsibilities:
The Physician Assistant (PA) provider is an integral member of the palliative care consultation team at MJHS Hospice and Palliative Care, collaborating with physicians, nurse practitioners, and interdisciplinary staff to provide expert symptom management, advance care planning, and hospice eligibility evaluations. The PA works in various settings, including hospitals, skilled nursing facilities, and the community, delivering patient-centered, goal-directed care for individuals with serious illness. The PA functions with clinical autonomy consistent with state licensure and organizational protocols, independently conducting comprehensive assessments, treatment planning, and follow-up care.
Initial patient encounters are performed in response to consultation requests, with ongoing visits as appropriate. The PA ensures care plans minimize illness burden and align with patients’ goals and values. The role includes participating in on-call rotation, documentation, interdisciplinary collaboration, education, and quality improvement initiatives to support high-quality palliative care delivery.
Qualifications:
• Master of Science in Physician Assistant Studies from an accredited program required.
• Minimum 1–2 years clinical experience as a licensed Physician Assistant preferred, with a focus in palliative care, hospice, geriatrics, or related fields.
• Experience in skilled nursing facilities or community-based care is preferred.
• Active Physician Assistant license in New York State.
• Current DEA registration allowing prescription of controlled substances.
• Certificate of Added Qualifications (CAQ) in Hospice and Palliative Medicine preferred.
• CPR/BLS required.
• Valid New York State driver’s license preferred.
• Proficient in comprehensive assessment and management in advanced illness.
• Skilled in conducting advance care planning and goals-of-care discussions.
• Proficient in hospice eligibility criteria and recertification processes.
• Ability to coordinate care within interdisciplinary teams to deliver holistic patient and family support.
• Demonstrated critical thinking and clinical decision-making capabilities.
• Proficiency in Microsoft Word, Excel, and electronic health records (EHR).
• Excellent communication, teaching, and mentoring skills.
• Competent in medical documentation, coding, and regulatory compliance.
• Ability to function in a culturally diverse environment and work non-judgmentally with patients and families of any background.
• Ability to work flexible hours, travel within the service area, and manage competing priorities.
