Responsibilities:

Overall responsibility for developing new referral relationship opportunities for perspective hospice patients; acting as a resource to these accounts and existing accounts in an assigned territory. 

Qualifications:

  • Graduate from an accredited School of Socia Work MSW.
  • Experience in Hospice or Palliative Care preferred and experience in Account Management.
  • Knowledge of COPs for Hospice and Palliative Care and reimbursement for Hospice desirable.
  • Minimum of one-year experience in outreach, sales or marketing.
  • CHPN after first year of hire.
  • Ability to work within an interdisciplinary team environment  
  • Excellent public speaking abilities.
  •  Well-developed teaching, public speaking, leadership, and organizational skills
  •  Able to work creatively within budgetary constraints
  •  Goal Oriented Change Agent
  •  Excellent verbal and written communication skills
  •  Advanced program development skills
