Responsibilities:

Evaluates patients referred for home health services. Manages and facilitates the transition of patients onto the home care program in accordance with applicable law, regulations and agency policies and procedures.

Qualifications:

  • Associates in Nursing; Baccalaureate Degree preferred.
  • Minimum of one year of acute, Medical-Surgical nursing experience, preferred.
    Home Care experience preferred
  • Specialized knowledge in Microsoft Office, (Word, Excel, Outlook, etc.)
