Assistant Director of Hospice Clinical Practice is a leader for hospice and palliative care operations/Clinical with total accountability for successfully implementing, achieving and sustaining the MJHS Hospice and Palliative Care mission, vision, and strategic plan for quality/compliance outcomes.
This Assistant Director of Hospice Clinical Practice is a nurse that leads by exuding the core values of MJHS Hospice and Palliative Care and inspires and motivates all staff to be “world class”. The Assistant Director reports to the Director of Clinical Services and works closely with the management team.

Responsibilities:
- Creates, revises and communicates policy reviews and updates for management and staff. Educates staff as needed with policy & procedures manual.
- Communicates with all directors and managers in disseminating policies from the Director of Clinical Services, facilitating the process of Clinical Practice Council.
- Assuring continuity of care for all patients, involving any and all departments necessary to insure quality and continuity of patient/family care.
- Assures that the teams utilize appropriate resource services (ambulance, IV therapy, DME, inpatient facilities, and resource agencies) to implement orders.
- Assures the care delivered in a hospital, long term care facility, Residences, In-Patient or Assisted Living Facility is appropriate, meeting the needs of the patient/family as well as the facility in accordance with the goals set forth by the Director of Clinical Services.
- Plans, organizes, and communicates with the Director of Clinical Services/management staff in order to meet and exceed the needs of the patient/family, referral sources and the community with complete customer satisfaction.
- Assists the clinical team in the provision of quality patient care services within the business model (The Model) and productivity standards, reporting in with ED as applicable.
- Assists with the innovation of new programs, when appropriate, to better meet the needs of specific patient populations.

- Assists the Director of Clinical Services and the management team with educating staff in all areas of federal, state and local laws and regulations as well as Joint Commission standards and CMS conditions of participation and the Agency’s standards of care, policy and procedures.
- Conducts chart reviews for appropriate documentation of patient/family care and utilization of resources as directed by the Director of Clinical Services.
- Works with the Vice President of Clinical Services on special projects interfacing with all aspects of the organization in collaboration with management.
- Manages performance of direct and indirect reports. Provides orientation, coaching, training and development. Takes corrective action when necessary.
- Utilizes corporate services resources such as Human Resources, Compliance, Legal, Finance, Information Services, Marketing, etc., as appropriate.
- Ensures appropriate staffing and resources to support department/agency services. Monitors productivity throughout the year and participates in annual budget preparation.
- Interviews and participates in the selection process for qualified staff in collaboration with Human Resources and department management.
Qualifications:
- Registered Nurse (RN) in the state of New York.
- Master’s degree from an accredited four-year college or university or an equivalent combination of education and experience: Nursing, healthcare administration or related field preferred.
- Hospice and palliative care experience strongly preferred.
- A minimum of five years’ experience in healthcare, with three years managerial/administrative experience required.
- Knowledge and experience in healthcare management and interpersonal relations.
- Knowledge of current Joint Commission accreditation program.
- Communicates effectively, with excellent written, verbal and listening skills.
- Employs planning, business and financial management skills, including evaluative skills.
- Collaborates effectively as a leader in a dynamic healthcare setting.
- Employs problem-solving, teamwork, and consensus building and negotiation skills.
- Prioritizes work activities to achieve goals in a timely manner.
- Demonstrates understanding of the needs of patients with serious progressive illness and their families.
