Responsibilities:

  • Assess the client's medical, mental health and financial strengths and weaknesses for dare planning in a culturally sensitive and client — strength based perspective.
  • Establish referrals needed for Meals on Wheels (MOW), Social Day Care, and EISEP In-home Services and on-going (in-home visits) according to the authorized care plan which includes service coordination and monitoring of services, including initial follow-up, and 15 days review of each authorized DFTA service, with one phone call every two months or 3 months for MOW or CM ONLY services. This may include filling out applications, providing counseling and education, securing concrete services, crisis intervention, home visits, referral to the Independent nurse consultant, or follow up and monitoring over a course of time.
  • Inputs information directly into DFTA's PDS computer system. Completes required forms for services provided direct and in-directly to client care services and coordinated activities.
  • Provides case management services based on level of client acuity and other service requirements. Identifies clients at risk for hospitalization or falls and works with interdisciplinary service team and community services to resolve or minimize risks. Assists client/family in emergency disaster planning. Maximizes
                                                                                

Qualifications:

  • Four years full-time experience in social casework, in social work in a community or social action program, teaching in an accredited school or as a community services worker or case aide in a human services agency;
  • Preference to bilingual staff : Spanish and Russian as indicated by populations to be serviced.
