Responsibilities:

The Patient Representative plays an integral role in improving the patient/family experience while on hospice service.  The Patient Representative provides supportive calls to the patients and families at end of life, maintains patient experience data and communicates patient/family needs and concerns to the teams and Quality Review Coordinator. 

  • Manages coordination of calls as initiated by workflow and as initiated by primary members of the IDT, noting need to follow up with phone calls and visits.
  • Participates and contributes to the Interdisciplinary Team Process.
  • Displays professionalism during all communication with patients, families and co-workers. 
  • Reports all Patient Experience Program shift occurrences and complaints.

Qualifications:

  • High School Diploma or equivalent required

  • Prior experience working within a healthcare environment, hospice preferred knowledge of hospice HIS requirements

  • Excellent verbal and written communication skills

  • Use of various computer programs including Microsoft Word, Excel, and Power Point
