Responsibilities:
Do you have a "Hospice Heart"?
If your answer is yes, then you are just the nurse we are looking for. When days cannot be added to a life, we add life to remaining days. Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.
- The NP Provider Hospice/Palliative Care has two primary responsibilities. They are one of a group of MDs and NPs who staff palliative care consultation services that function as programs of MJHS Hospice and Palliative Care. Additionally, they will provide NP-appropriate service to patients enrolled in hospice
- NP Providers may work in a nursing home, hospital or community environment
- Ideal candidate will have specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and the ability to work independently or as part of a treatment team
- In all settings, they will provide in-person visits or telehealth visits, as appropriate
- When working as a palliative care provider, initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family
- Each of these encounters comports with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
- In the role as hospice NP, they will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
- NPs also will contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
- In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
- Other activities related to quality improvement projects also may be required, as appropriate.
Qualifications:
- Master's degree in Nursing and graduate of an approved Nurse Practitioner Program required
- Minimum of 1 year clinical experience as a NP provider required
- Prior experience working as a member of a specialist-level palliative care team or working in a hospice environment preferred
- License/Certification: New York State RN licensure, Certified Nurse Practitioner, CPR-BLS
- Valid NYS Driver's license preferred
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire
