Located in the heart of the community of Washington Heights in Northern Manhattan is a 705-bed rehabilitation and nursing care facility with a mission to provide quality care through diverse programs and services designed to support health and independence to individuals needing short-stay or long term inpatient care.

Responsibilities:
Supervises Senior Service Specialists in their provision of psychosocial and concrete services through individual and group work modalities. Helps Senior Service Specialists to set work priorities and maximize agency and community resources in the provision of case management services.
Assists Senior Service Specialists to assess client's needs and provide the client with information and community referrals to promote independent self-care and safety in the community. Works towards resolution of identified client problems through counseling, case conferences, and authorization of DFTA services of Meals on Wheels (MOW), In-home Services and/or Social Day Care Services.
Evaluates personnel performance through observation, record review and home visits. Instructs and assists in promoting more effective performance. Reviews documentation and visit reports from Senior Service Specialists for completeness and quality of service. Audits records to ensure that care and coordination is being provided in compliance with DFTA Regulations and to meet the client needs.
Assists in the recruitment and hiring of staff Senior Service Specialists; Orients and teaches new Senior Service Specialists or delegates this role to appropriate personnel.
Approves initial and ongoing re-assessments, and discharges. May initiate and coordinate case conferences working with expanded service team of Intake Liaison, Assist. Director, and others within the organization and as appropriate external care providers and client/family. May provide direct casework services to community clients on a case-by-case basis.
Helps to plan in-service and education programs. Participates in performance improvement activities.
Evaluates need for additional client services. Promotes utilization of community resources and acts as a liaison with other community agencies providing services. Attends conferences and appropriate community meetings to improve case management skills and for informational purposes.
May be responsible for 24-hour on-call as indicated by staffing needs of the clinical service team and part of rotation schedule for emergency disaster planning.
Performs other duties, as requested.
Qualifications:
A Master’s Degree in Social Work, Human Services, or Social Sciences from an accredited college.
Two to three years of supervisory experience in Case Management.
Bilingual (Spanish) a plus. Phone intake experience a plus.
