Responsibilities:

Working on-site at a major medical center, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice services. The Hospice Care Planner will facilitate admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members. Must have extremely strong Clinical experience.
