Located along a scenic waterfront property in Brooklyn, NY, our Menorah Center for Rehabilitation and Nursing Care is seeking a Physical Therapist to evaluate, assess and treat patients within our rehabilitation center.

Responsibilities:


• Evaluate residents to assess appropriate Physical Therapy treatment and rehabilitation potential

• Establish Appropriate Treatment goals and Plan of treatment for each resident.  

• Plan and administers treatment modalities in accordance with a resident plan of care
• Documents screenings, evaluations, progress notes and care plan tracking forms
• Maintains statistical data on patients
• Provides guidance and supervision to assigned Physical Therapy Assistants

Qualifications:

• Bachelor's degree in physical therapy
• Current NYS Physical Therapy license   
• 2 years of geriatric experience from a rehabilitation & nursing home facility is preferred
