Transitional Care RN - Team members in this managed care nursing job act as a patient advocate in facilitating, coordinating and communicating the transition of patients to next level of care in accordance with applicable law, regulations and agency policies and procedures. Works in a collaborative role with MJHC and select inpatient facilities to evaluate inpatients referred for transitional planning and/or home health services.

Quality Review Specialist - Team members in this managed care nursing job are responsible for quality assurance, accuracy and overall integrity of the care management records and documentation completed by Care Management staff. This role is to ensure compliance with NYS DOH and CMS regulations through development of auditing tools, specifically validating all data collected.
